Rights
- Receive services without discrimination including but not limited to race, color, religion, ethnicity, disability, medical condition, gender, age, marital/relationship status, gender identity or sexual orientation.
- Be treated with dignity and respect by all ESS employees and representatives.
- Be safe from harm or threats of harm while receiving services from ESS.
- Refuse or terminate services to the extent permitted by law or court order.
- Privacy and confidentiality of personal and identifying information and the right to provide informed consent and statement of exclusion prior to release of any private information to a third party to the extent permitted by law or court order.
- Review their case record upon reasonable request and in the presence of agency personnel, add a statement to their case file, and be informed of any responses to client statements added to files.
- Participate in all decisions around service or treatment plans and receipt of services from ESS.
- Receive high quality services from trustworthy ESS staff, who conduct themselves professionally and ethically, in accordance with the highest standards of the profession.
- Receive prompt response from ESS staff when leaving a phone message or raising a question or concern either in conversation or in writing.
- File a grievance with ESS Management, ESS HIPAA Privacy Officer, or governing agencies if not satisfied with services.
- Request a new worker after a review meeting with the Supervisor and Program Director.
- Be informed if it is necessary for ESS staff to make a report of suspected abuse or neglect to the authorities, unless such report would place a child or other individual in imminent danger.
- Be informed of any conditions which could result in termination of services.
- Exercise their rights without fear of retribution.
Responsibilities
- Treat ESS staff in a respectful manner
- Behave in a way which allows other clients and staff at ESS to be safe from harm or threats of harm.
- Inform your worker if you do not understand any part of your service/treatment plan or any request from ESS staff.
- Inform your worker of all information about you and your family which is relevant and important to the services you will receive from ESS.
- Attend all appointments with ESS staff or call to reschedule if another appointment or emergency prevents you from attending your appointment.
- Inform your worker of any changes to your phone number, address, or family situation which might affect services.
- Participate actively in the development of your service/treatment plan.
- Report any problems or concerns to the chain of command if you are not satisfied with the services you receive from ESS.
