The Adoption Case Planner will work with Adoptive Resource Parents towards achieving the goal of Adoption.
Complete and forward Adoption Packages to Adoption Attorneys, Complete applications for Adoption Subsidy Packages, Complete Psych-socials as needed, Conduct monthly in-home visits with foster children and Adoptive Parent(s), Complete monthly progress notes and FASP, Prepare Permanency Hearing Reports/Update Reports and attend court, Participate in Family Team Conferences/Crisis Meetings, Arrange and collaborate with Clinic staff to address children’s on-going mental health needs

The Case Worker will expedite reunification by providing needed services and advocacy to parents and children temporarily separated because of abuse or neglect.
Conduct monthly safety assessments of foster and biological homes; Interview parents and children in the agency, at the foster and biological homes, and in community facilities, Schedule and supervise family visits in the agency and general community, Document in the State Connections database, each contact with a child, birth and foster parent as well as service providers, Collaborate with medical specialists to meet health care needs of children; complete FASPs, Plan amendments and other related reports.

The Visitation Specialist will be responsible for ensuring that the children’s overall needs are met and that each child is appropriately placed to avoid and alleviate any risk to their overall needs.
Coordinate and arrange/child visits, Provide support to the birth parents to encourage active participation during the visits, Work with the birth parents on enhancing their interaction during the visits, Provide various recreational activities in order to strengthen parent/child bond, Record all progress notes in connections on weekly basis, Prepare reports for court regarding assessments of parent/child visits, Escort child/ren to visit their parent through Chipps program if necessary, Attend parent to parent conferences for new cases
BA/BS in Human Services or Social Work preferred; Bilingual Spanish required; Vigorous commitment to community work related to advocating for child’s educational needs; Ability to be team play and work independently; Good verbal and written communication skills
The LPN will help provide pediatric care for children in Foster Care and Adolescent Residential Care populations within the agency’s programs.
Obtain and document vital signs, Assist MD and/or PNP in provision of physical examinations and immunizations, Ensure that medical record contains written documentation of all health and mental health services received, Ensure documentation of semi-annual dental visits.
Current New York State LPN License; Bachelors Degree in Health Service, Biology or related field required; Bilingual Spanish required; 2 years clinic experience working in pediatrics with knowledge of or experience in foster care, management of medical records, and health care case management desired; CPR Certification and certified to perform pre and post HIV test counseling,
Maintenance Worker (2 Positions)
The Maintenance Worker is responsible for the maintenance of agency facilities as well as providing handyman services.
Provides handyman services such as building shelves, repairing furniture, hanging fixtures and painting as needed, Cleans assigned office areas, bathrooms, kitchen areas, Sets up conference rooms for meetings and provides refreshments, Assists Management in conducting inventory of supplies, Removes snow from front entrance of facility
H.S. Graduate; 2 years experience in office maintenance or handyman work; Basic mechanical, plumbing and electrical knowledge; Attention to detail; Valid driver’s license (must have own vehicle); Ability to work in team environment, Flexibility in schedule
The Program Aide supervises and guides assigned participants through a variety of academic, recreational, youth development and cultural activities within a safe, wholesome environment. The Program Aide will enrich each participant with a variety of positive assets that will help produce a well-rounded individual and better equip them for the long term challenges of good citizenship.
Supervise an assigned group of participants; Assist participants with daily homework assignments; Select and conduct organized sports, games, arts and crafts and other program activities; Maintain order and cleanliness of program areas.
High school diploma; Prior experience working with school age (6-12) children; Ability to assist participants with homework; Ability to engage participants in a variety of activities.
The Activity Specialist will supervise and engage participants in specific specialized activities (ex. Arts, dance, sports etc.)

High school diploma; Prior experience working with school age (6-12) children; Ability to engage participants in a variety of activities; Proof of specialty or ability to conduct specialized activities.
The Direct Care worker provides hands on care, assistance, training, and support to each individual who lives in the facility.
Evaluate clients to assess risks and develop case plans; Become a consistent helping person in the individuals lives; Learn each individuals way of functioning and know when the individual requires help; Teach and assist each individual the skills needed for daily living and socialization; Complete data on daily logs, activity sheets, trip evaluations and attendance sheets; Participate as a member of a Team in the development, implementation and evaluation of each individual’s Plan; Carry out fire drills, evacuations and emergency procedures.
Minimum requirement High School graduate; 1 year experience working in the Human Service and/or Mental Retardation, Developmental Disabilities field; Ability to relate and work with consumers with mentally retardation and developmental disabilities; Successfully passed and maintains AMAP certification; Successfully passed and maintains First Aid and CPR (Red Cross) certification; Successfully passed and maintains SCIP certification; Meets all the requirements on the Annual Performance Assessment; Valid New York State Drivers License is required.
The Child Care Worker provides supervision and care to children with their daily routine, and create a warm, safe environment.
Manage group, understand peer dynamics, create and maintain a positive peer group culture; Provide attention and support of medical issues, students educational/vocational needs, casework and clinical appointments; Develop healthy and helping relationships with children and their families; Coordinate individual activities and group activities such as sports, games, trips; Log all reports, incident reports, and inform the social worker and supervisors of any issues related to children; Provide basic care; health, safety, meals, snacks.
High School Diploma or GED required; Experience working with children and some college preferred; Have a strong sensitivity to cultural differences present among staff and clients within our organization; Possess a strong belief in people’s ability to grow and change; Forge a mutually respectful partnership with persons served and their families.
